Old man suffering from complex heart disease saved at Narayana Hospital

KATRA, July 15: Shri Mata Vaishno Devi (SMVD) Narayana Superspeciality Hospital has achieved another milestone by saving an 82-year-old suffering from a complex and rare coronary condition through a single intervention.
Hailing from Bakshi Nagar (Jammu), the patient was brought to the Hospital with acute chest pain. In the initial assessment, the patient was found to be having inferior wall myocardial infarction, a condition that leads to decrease in the blood flow to the heart and thereby damaging the heart muscles.
Emergency Coronary angiography was performed on the patient which revealed that he was also having anomalous right coronary artery origin from left coronary sinus with 95% stenotic lesion in mid part. This is a rare congenital coronary anomaly which occurs to less than 1.7 % of all patients. Patients with such coronary anomalies usually suffers from Ischemia (Ischemia is a restriction in blood supply to tissues, causing a shortage of oxygen that is needed to keep tissue alive) as the result of mechanical compression of artery against the great vessels of the heart.
In this case it was the mixture of mechanical compression, atherosclerosis and plaque rupture with subsequent development of myocardial infarction making the case a complex one. While Coronary Artery Bypass Surgery is the most accepted treatment modality, but owing to the age and the co-morbidities that the patient had, doctors in consultation with the patient’s family opted for angioplasty. Doctors performed the angioplasty and inserted a stent into right coronary artery through the hand. Today, the patient is leading a healthy life.
Speaking about the case, Dr Sushant K Sharma, Consultant- Cardiology, SMVD Narayana Superspeciality Hospital, said, “Angioplasty is a challenging procedure in elderly patient. For these subset of patients, angioplasty is usually done through leg as it requires special techniques and hardware. But in this case, as the patient was a senior citizen with multiple comorbidities, we had to do it through the hand. Intervention through hand requires special techniques and expertise. The procedure was successful and he has recovered from his condition.”
