Issuing duplicate passport

Sir,
There is no denying the fact that the Passport issuing authorities should exercise every type of caution and subject the aspirant for a Passport to all reasonable verification and scrutiny. It has to be more than adequately ensured that the Passport was being issued to a genuine person when it is the case of a sensitive state like Jammu and Kashmir for obvious reasons. It is also expected that the whole exercise , in the initial stage, should be completed in one or two weeks , that time being sufficient for the purpose.
The question is about an incident which can take place in normal course  when a pass port, got after going through all formalities, gets lost , stolen or misplaced. What is the procedure to get a duplicate one ? It is unbelievable that for getting a duplicate one, again the same lengthy procedure has got to be gone through as if it was going to be done for the first time for the applicant. It is true of our state and perhaps not in other states. Well, it goes against the logic and the reason and can well be termed as unnecessary and time wasting exercise which aims at delay, wastage of time,  non availability of the document of Passport  in moments of exigencies to be attended by the applicant and other hassles. It is requested that a new look at the issue needs to be had and issuing of duplicate passport made easy and less time consuming. In the same way, for obtaining visa, one has to go to Chandigarh, Ludhiana or Delhi , the nearby specified places and here also norms and procedure of intimation of granting of visa need to be simplified.
