Rare ballooning case done at Sudan Heart Care Centre

JAMMU, Oct 15: A deaf and mute lady – who is unable to speak or hear by birth underwent successful ballooning of mitral valve for the second time at Sudan Heart Care Centre yesterday.
Patient Babli Devi was suffering from Rheumatic Heart Disease which affects the valves of the heart causing their narrowing and/or leaking. Rheumatic heart disease occurs mainly in poor living conditions in early childhood.
This case was unique, as the patient had undergone ballooning in year 2002 done by Dr Dinesh Sudan, who is a renowned cardiologist and DM gold medalist from AIIMS. Usually ballooning relieves patient from symptoms for 10 years. However, patient Babli Devi remained well for 15 years, when she returned with symptoms of increasing breathing difficulty.
She was again assessed by Dr Dinesh Sudan, who was of the opinion that repeated BMV (ballooning of Mitral valve) should be done. However, he cautioned that repeated ballooning has a higher risk of valve rupture which can create an emergency. The patient then tried to get the procedure done at various multispecialty institutes but were refused.
Babli Devi was a special case because of difficulty in communication during the procedure and also because of frail constitution – with patient being just 40 kg of weight. So it was decided by the family to get ballooning done at Sudan Heart Care Centre by Dr Dinesh Sudan. However the procedure went off well and satisfactory results were obtained.
Dr Dinesh Sudan said this case was a challenge due to lack of communication with the patient which increases the difficulty level of the procedure. The procedure could have been under general anesthesia but there could be more complications. Ultimately, the patient’s sister who is also her care taker was stationed in the cath lab in scrubs (theatre dress) and procedure was completed with light sedation.
Dr Sudan said that balloon Mitral Valvotomy for Mitral Stenosis i.e. narrowing of Mitral valve is a standard procedure which has replaced the surgical treatment of same condition called closed mitral valvotomy i.e. CMV. Both give equivalent results but after ballooning there is no scar and minimal hospitalization when patient is discharged the next day.
