Attaining parenthood is one of the needs innate in every woman and man. When achieving this much desired goal becomes difficult, it gives rise a lot of suffering to an individual or couple. Infertility affects 15percent of the general population.Roughly 30 percent of cases are due to male factor, 30 percent female factor, 30 percent due to acombined male and female factor and 10% remains unexplained.
There is a definite increase in the number of couples approaching fertility clinics.This could be because of increased awareness and ever increasing number of fertility clinics. When pregnancy does not occur within the first few cycles, theypanic and seek medical help.Assisted reproduction would definitely be indicated in them.
There appears to be an increasing incidence of low sperm counts in men. Variousetiological factors like environmental pollutants, increased stress, raised scrotaltemperatures due to laptops have been implicated.It is a well known factor that smoking, drinking and drugs have adverse affects on fertility potential. Same is applicable to grossly underweight and overweight situation.
IVF- In-vitro fertilization = fertilization outside the body ………….. Forming an embryo
* Meeting of the sperm and oocyte (egg) outside the woman’s body, in a dish and subsequently getting fertilized. The developing embryo is then placed inside the uterus.
Aim:
Large number – follicles
Manipulate events in h-p-o axis
WHEN – IVF?
* Blocked fallopian tubes
* Severe abnormalities semen analysis.
* “Unexplained infertility”.
* Endometrioses
* Genetic diseases in either parent resulting in miscarriages/stillbirths.
* Women whose ovaries are failing due to age or other cause. Such women undergo IVF with donated oocytes.
SELECTION OF PROTOCOL- AS PER PATIENT PROFILE
OUTCOME – IVF
PREGNANCY: 30-40%
* Considered high-risk because of the mode of conception.
* Pregnancy problems – same as natural or IUI conception
